We're looking for a compassionate and experienced Liaison/Triage Practitioner to join our established Liaison Nurse Team (LNT), combining specialist mental health support with a key role in delivering the Northeast and North Cumbria (NENC) Neurodevelopmental Triage Service.
This is a varied clinical role where you'll work both independently and as part of a multidisciplinary team to provide expert clinical advice, assess and manage risk, support safeguarding processes, and help ensure patients receive the right care at the right time.
As part of the Liaison Nurse Team, you'll support patients across Psychiatry UK's services, using your clinical expertise to assess complex mental health presentations, provide guidance to colleagues, and contribute to safe, high-quality, patient-centred care.
Alongside this, you'll play an important role in the delivery of the NENC Neurodevelopmental Triage Service—a clinically led, digitally enabled service designed to improve access to support and assessment for adults referred with possible neurodevelopmental conditions. Using evidence-based clinical judgement, you'll undertake holistic triage and risk assessments, considering neurodevelopmental presentations, mental health needs, safeguarding concerns and functional impact.
Working closely with NHS partners, accredited providers and internal clinical teams, you'll help patients access the most appropriate support, treatment, diagnostic assessment or alternative care pathway. Your work will be central to ensuring people receive timely, equitable and person-centred care in line with national guidance and local commissioning priorities.

This is a home-based role (applicants must reside in the UK), though occasional travel may be required for face-to-face meetings at various locations within the UK.
As our Liaison/Triage Practitioner you will:
Provide expert clinical support and advice to Psychiatrists, Prescribers and the wider multidisciplinary team.
Assess patients' mental state, clinical presentation and level of risk through telephone and remote consultations.
Deliver timely clinical triage and risk assessments in line with Psychiatry UK policies and agreed care pathways.
Triage referrals for neurodevelopmental assessments using evidence-based clinical and functional criteria.
Make autonomous clinical decisions, ensuring safe, effective and person-centred care.
Develop and implement appropriate risk management plans, escalating concerns and referring to external or emergency services where required.
Maintain accurate, timely and comprehensive clinical records within electronic patient systems.
Promote shared decision-making and provide patients with information, advice and support regarding assessment, treatment and onward care.
Work collaboratively with internal teams, NHS partners and external agencies to ensure coordinated patient care.
Contribute to the development, evaluation and continuous improvement of clinical services and care pathways.
Undertake safeguarding assessments and ensure appropriate action is taken in line with organisational policies.
Prioritise and manage a varied clinical workload while maintaining high standards of care and professionalism.
Maintain up-to-date knowledge of relevant legislation, national guidance, evidence-based practice and professional standards.
Participate in clinical supervision, mandatory training and continuing professional development to maintain professional competence.
Promote the values of Psychiatry UK, supporting colleagues and contributing to a positive, collaborative and patient-focused culture.
To succeed as our Liaison/Triage Practitioner, you will:
Demonstrate and maintain a high level of relevant clinical skill and knowledge appropriate to the role.
Maintain and update mandatory and statutory training as set out by Psychiatry UK.
Maintain all professional registrations that allow continuation of for Registered Nurse/Occupational Therapist practice requirements.
Participate in suitable professional development opportunities.
Promote and develop the role of the Liaison/Triage Practitioner within the wider P-UK team.
Access regular professional and clinical supervision made available within Psychiatry UK.
Essential Qualifications, Skills and Experience:
Registered Nurse (NMC) or Occupational Therapist (HCPC) with active professional registration.
Minimum of 3 years post qualification experience in relevant mental health setting or demonstrable evidence of experience of safe independent clinical decision making in a mental health setting.
Evidence of Continued Professional Development (CPD).
Relevant experience of mental health care provision and/or neurodevelopmental conditions.
Relevant experience of mental health treatment and management.
Ability to undertake holistic clinical triage and risk assessment and apply evidence-based decision-making.
Understanding of safeguarding, functional impairment and the impact of co-occurring mental health conditions.
Ability to work autonomously and effectively manage own workload.
Advanced IT skills sufficient for remote working and online service delivery.
Evidence of effective multidisciplinary and multi-agency working.
Highly developed communication and interpersonal skills.
Good knowledge of legislation and developments relevant to ADHD, autism and mental health services.
Ability to adapt to the changing needs of a developing service.
Enhanced DBS clearance (can be provided by P-UK but you will be expected to register with and pay for the DBS update service).
Ability to motivate and organise self to ensure accessible, compassionate and high-quality service provision.
Desirable Qualifications, Skills and Experience:
Relevant postgraduate qualification or additional training in mental health, neurodevelopmental conditions, or psychological therapies.
Experience working within ADHD and/or autism assessment, treatment or ongoing care services.
Experience of working within a remote or digital healthcare environment.
Previous experience within liaison psychiatry, community mental health, crisis services, or primary care mental health.
